Returning Students:
Log in to your Family Skyward account to complete the online enrollment. If you do not have access to a computer or the internet, there will be computers available for use at BES and BJSHS on the scheduled enrollment dates. If those dates don't work, please contact the school to make other arrangements.

Transportation
If you would like bus transportation for your student(s) to and from school, you must complete this form.
Students who are currently being transported must reapply for the new school year.
If you live within the city limits of Beloit, you are not eligible for transportation.

Health Information
Health Assessments
For students entering school for the first time.

Kansas requires a health assessment for students ages 9 and younger who are enrolling in a Kansas school for the first time.
Immunizations
Help ensure your student's immunization records remain current. Updated records should be provided to the school nurse as soon as vaccinations are received.

Attention parents of 6th grade students: Students entering 7th grade are required to have a Tdap vaccine to attend school.

Medication at School
Students who require medication during the school day must have the appropriate authorization forms on file. Prescription medications require authorization from both a parent/guardian and a physician, while over-the-counter medications require parental permission. All medications must be provided in their original container.

Action Plans & Emergency Forms
If your child has a chronic health condition or life-threatening allergy, please notify the school nurse so an appropriate care plan can be developed before the school year begins. Updated forms are required annually.
