Postsecondary Visit Permission Slip

If you are planning to take a visit to a post-secondary institution, you must turn in this completed permission slip to Mrs. Eilert 24 hours in advance in order for the visit to be an excused absence.

NCAA Eligibility Center

NCAA Eligibility Center:  If you are interested in playing sports at the collegiate level in a Division 1, 2, or 3, you need to be registered in the NCAA Eligibility Center (formerly known as the NCAA Clearinghouse). You can register with the NCAA Eligibility Center at:

Registering with the Selective Service

ACT Scores for Concurrent Credit Classes

If you would like to take College Algebra as a junior or senior, you must meet one of the multiple measures below:

Math ACT: 22 or higher OR
Math SAT: 540 or higher OR
ALEKS PPL: 46 or higher OR
Accuplacer QAS: 263 or higher OR
HS GPA and Course Grade: 3.25 cumulative GPA (unweighted) and B- or higher in Second semester Algebra 2 or Integrated Math 3 OR
Institutional Measure* (including professional discretion)

If you would like to take College Essential Math or College Statistics as a junior or senior, you must meet one of the multiple measures below:

Math ACT: 19 or higher OR
Math SAT: 510 or higher OR
ALEKS PPL: 30 or higher OR
Accuplacer QAS: 255 or higher OR
HS GPA and Course Grade: 3.00 cumulative GPA (unweighted) and C- or higher in Second Semester Algebra 2 or Integrated Math 3 OR
Institutional Measure* (including professional discretion)

If you are hoping to take College Calculus, you must have an ACT math score of 25 or above and have had College Algebra as a prerequisite.

If you are planning to take Composition I/Composition II, then you must have an ACT English score of 20 or above.
